A 52-years-old man reported progressive dyspnoea of more than 4 months. He could not walk more than 200 m and he was hospitalized due to worsening heart failure 2 weeks prior to examination. He was treated with an ACE inhibitor and a loop-active diuretic (furosemide 40 mg, captopril 12.5 mg). After demission he was admitted to our outpatient clinic in order to discuss the indication for elective cardiac transplant. Other relevant diseases in the past were not reported. The patient smoked 15 cigarrettes per day; drinking of alcohol was not permitted. 781b155fdc
